Everyday clinical care
Day-to-day outpatient care that catches problems early and keeps long-term conditions under control, so fewer patients reach crisis.
- General outpatient consultations and follow-up visits
- Vital signs monitoring and clinical assessment
- Wound care and dressing, including diabetic foot ulcers
- Follow-up of hypertension, diabetes and asthma
- Basic laboratory tests, with referral for advanced investigations
- Medication adherence, nutrition and lifestyle counselling

Community outreach and education
Health knowledge and early detection brought to where people live, so patients are found and supported sooner.
- Health education in communities, schools and faith groups
- Blood pressure and blood glucose screening camps
- Palliative care awareness to counter myths and stigma
- Working with local leaders and village health teams to identify people who need care

Referral and linkage
What a community clinic should not do alone, we coordinate. Patients reach the right level of care, and come back to us for continuity.
- Referral to hospital for imaging, surgery, fracture care and admission
- Linkage to cancer services for diagnosis and treatment
- Referral for specialist consultation and advanced laboratory tests
- Back-referral: continued care at home after hospital discharge
- Referral notes and follow-up, so no patient is lost between services
Referring a patient to us
Hospitals, clinics and village health teams can refer patients who need palliative care close to home. Call or send a WhatsApp message with:
- the patient's name and condition
- where they live in or around Gulu
- a family member's phone number
- the discharge summary, if there is one
We will contact the family and arrange to see the patient.
